PRIMARY CARE:

We are accepting new Primary Care patients. Your primary care provider is responsible for continuing comprehensive care, including prevention, health maintenance, chronic care management, and annual wellness visits. Please complete the new patient paperwork. The form will be automatically emailed to us and we will call to schedule your appointment as soon as possible.

URGENT CARE:

We are accepting new patients for Urgent Care. Urgent Care is reserved for sick and injured patients and does not provide continuing comprehensive care. Urgent Care is provided on a walk-in basis and paperwork can be completed at the office upon arrival, no call or appointment is necessary. We accept walk-ins Monday – Friday 8am – 5pm.
